  • This warning banner provides privacy and security notices consistent with applicable federal laws, directives, and other federal guidance for accessing this Government system, which includes (1) this computer network, (2) all computers connected to this network, and (3) all devices and storage media attached to this network or to a computer on this network.

Privacy Act Statement

  • General - This notice is provided pursuant to the Privacy Act of 1974 (5 U.S.C. Section 552a) for individuals supplying information as data input to the FDA's USA Learning system. 
  • Authority – Information you submit to participate in training and learning activities is being collected under the authority of 5 U.S.C. §§ 301, 3301, 4115 and 4118 for records about FDA civilian employees; 42 U.S.C. §§ 202-217, 218a, 224, 228,233, and 410(m) et seq. for records about HHS Public Health Service (PHS) Commissioned Corps employees; and 44 U.S.C. § 3554(b)(4) for any security and privacy training records about regulatory personnel.  
  • Purposes - The information entered into this data system becomes a part of the FDA Center for Tobacco Products (CTP) USA Learning system. The primary use of the information is to enable CTP employees to register for trainings, view e-Learning content, fill out training request forms SF-182 (eSF-182 to refer to electronic versions of the form), and have an individual record of completed trainings.  
  • Disclosures – FDA may disclose records from these systems internally to HHS personnel who are performing authorized duties in support of FDA training activities and/or administering the USA Learning system.  FDA may also disclose records from the system to government (Federal, State, and local) and non-government (private vendors of training courses or programs, private schools) training facilities for training program activities including crediting of work experience and/or verification of employment, training status and income. A full list of routine use disclosures is set forth in the Government-wide System of Records Notice (SORN) titled OPM GOVT-1 General Personnel Records and in the Department of Health and Human Services SORN titled 09-40-0001 Public Health Service (PHS) Commissioned Corps General Personnel Records, HHS/PSC/HRS. 
  • Effects of Nondisclosure - Providing the personal information requested is voluntary. However, failure to provide this information may result in limited access to resources for professional development and training.
